Please download this form and fill it out for each of your students, and include all the events they are participating in. This is different than the original form I had you fill out, this is the OFFICIAL district form. Please download and return ASAP no later than Sunday 03-29-09

Catalyst Youth Ministry, seems like only yesterday we were coming back from camp… You won’t want to miss this awesome time at camp. Signup sheets are available at the welcome center and during youth.
When: July 6-10th
Cost: $175  Camper (Deposit of $80 due with application)
Application: DUE SUNDAY MAY 3rd!!!!
